What You Will Do: Has as primary duty to drive robust integration of community partners (PCPs, Specialists, SNFs, Urgent Care etc.) with UM Shore Regional Health strategy and efforts to improve quality, assure a superior patient experience and ultimately demonstrate measurable reductions in Total Cost of Care. Stay abreast of trends all Total Cost of Care / Population Health policies & best practices from HSCRC, other UMMS hospitals etc. Succinctly communicate major changes to Population Health Office leadership and contribute to yearly strategic planning based on updates. Be main point of contact for Care Transformation Organizations in Maryland (e. g. UMMS QCN) and Managed Care Organizations that coordinate outpatient care for SRH patients. Directly supervise activities of Transitional Nurse Navigators. Other duties as assigned or requested.
